File Claims

See Federal Bankruptcy Rule 3004 and 3005 regarding filing of claims by the debtor or trustee or guarantors of co-debtors, etc. 

Instructions: 

  1. Complete Proof of Claim (B10).
  2. Select Bankruptcy
  3. Select File Claims
  4. Ensure correct case name and number match the document you are filing.
  5. Search for appropriate creditor from drop down list. Verify that the creditor's address is the same as the address on the Proof of Claim Form. 
    If creditor is not listed or different address, Add creditor
    CREDITOR ADDED MUST BE EXACTLY AS IT IS ON THE CLAIM FORM!!
  6. Enter all information in the appropriate fields for the claim you are filing.
    Amends a Claim # - BYPASS THIS FIELD Unless Amending Previous Claim
    Filed by: Select appropriate Party Filer. (i.e., creditor, attorney, trustee, etc.)
    U.S. Trustee ONLY: If filing an "Administrative" claim, select "trustee" as Party Filer.
    Unsecured, Secured, Priority
     U.S. Trustee ONLY: If filing an "Administrative" claim, select "Priority."
    Select type of claim and enter dollar amount(s). 
    Description or Remarks - Type further description or remarks for the claim.  
  7. Browse, verify and attach the Proof of Claim (B10) (pdf file.)
    Attach supporting documents, if applicable.
  8. Save the Notice of Electronic Claims Filing.

Add Creditor:

  1. Select Add Creditor 
  2. Enter creditor name and mailing address in the appropriate fields.
  3. Select type of creditor: Creditor or Administrative.
    U.S. Trustee ONLY: Do not select "Administrative" as creditor type.
  4. Select "yes" if on Creditor Committee.
  5. Last Entry
    Click on  "Last Entry" 
    Click on "File a Proof of Claim" hyperlink to select the creditor just added and to file the proof of claim (pdf file.)
  6. ~Note: Additional creditors
    Click on "Continue." 
    Click on the "Creditor Maintenance" hyperlink and repeat steps (Add Creditor 1 - 5.)

Add Individual Creditors

Instructions: 

  1. Select Bankruptcy.
  2. Select Enter individual creditors.
  3. Ensure correct case name and number match the document you are filing.
  4. Enter creditor name and mailing address in the appropriate fields.
  5. Select type of creditor: Creditor or Administrative.
    U.S. Trustee ONLY: Do not select "Administrative" as creditor type.
  6. Select "yes" if on Creditor Committee.

  4. Select Document Event: Assignment of Claim
  5. Check if joint filing with other atty,
    if not, select next.
  6. Select Party Filer.
    Party filer not listed, Add/Create New Party.
  7. Browse, verify and attach Assignment of Claim pdf file.)
    Attach supporting documents, if applicable.
  8. Enter :
    Claim # being assigned
    Assignee
    Amount
  9. Enter Claim # being assigned (second request)
  10. Pick Assigned Status
  11. Modify text, if applicable, such as:
    Title of document and/or
    Pertinent information - names, titles, property, etc.
  12. Warning!! Verify filing entry is correct before submitting the "Final Docket Text."
  13. Save the Notice of Electronic Filing

  8. Pick Transfer Type.
    Search for Transferee.
    Search for Transferor.
  9. Enter Claim #.
  10. Pick Transfer Status
  11. Modify text, if applicable, such as:
    Title of document and/or
    Pertinent information - names, titles, property, etc.
  12. Warning!! Verify filing entry is correct before submitting the "Final Docket Text."
  13. Save the Notice of Electronic Filing
