- A Mailing Matrix List of all creditors. You will need to enter your creditors on our website under FILING WITHOUT AN ATTORNEY>SUBMIT CREDITORS FOR MATRIX TO COURT.
- A Notice Required by 11 U.S.C. Sec 342(b) for Individual Filing for Bankruptcy Form 2010 . This form is usually included with the petition form.
- Filing Fee must be paid. See the Fee Schedule.
- The required forms will be provided to you via hyperlink in an email if you submit an eSR package and must be received within seven (7) days or your case will be dismissed. If you file your petition outside of eSR, please submit the forms below.
- Form 1007-1(G) Pro Se Filing Disclosure
- Form 2010 Notice Required by 11 U.S.C. Sec 342(b) for Individual Filing for Bankruptcy. This form is often included on most petition forms.

The documents listed below are required documents to be filed with the Voluntary Petition or 14 days thereafter.
