 Instructions for Individual Debtors (pdf)

 Required Documents Due WITH the petition (minimum filing requirements):
  • Form 101 Voluntary Petition for Individuals Filing for Bankruptcy (individual)
  • Filing Fee See Fee  Schedules  or
  • Form 103A Application for Individuals to Pay the Filing Fee in Installments (individual) ) $75.00 Minimum payment due with the Application
  • Local Form 1007-1(G) Pro Se Filing Disclosure (pro se individual)
  • Form 2010 Notice Required by 11 U.S.C. Sec 342(b) for Individual Filing for Bankruptcy (individual)
  • Mailing Matrix List of all creditors Local Rule 1007-I-1(E)
  • Local Form 1008-1 Verification to Mailing Matrix
  • Form 121 Statement About Your Social Security Numbers (pro se individual)
  • Form 119 Bankruptcy Petition Preparer’s Notice, Declaration and Signature (pro se individual with petition preparer)
  • Form 2800  Disclosure of Compensation of Bankruptcy Petition Preparer (pro se individual with petition preparer)

 Documents due with the petition or within 14 days:
  • Form 106A/B Property (individual)
  • Form 106C  The Property You Claim as Exempt (individual)
  • Form 106D  Creditors Who Have Claims Secured by Property (individual)
  • Form 106E/F  Creditors Who Have Unsecured Claims (individual)
  • Form 106G  Executory Contracts/Unexpired Leases (individual)
  • Form 106H  Your Co-debtors (individual)
  • Form 106I  Your Income (individual)
  • Form 106J  Your Expenses (individual)
  • Form 106J-2 Expenses for Separate Household of Debtor 2 (individual)
  • Form 106Sum Summary of Your Assets and Liabilities and Certain Statistical Information (individual)
  • Form 106Dec Declaration about an Individual Debtor’s Schedules (individual)
  • Form 107 Statement of Financial Affairs for Individuals Filing for Bankruptcy (individual)
  • Local Form 1007-1(H) Certification of Payment Advice's with attached copies of all payment advices (pay stubs) or other evidence of payment received by the debtor from an employer within 60 days before filing the petition (individual)
  • Form 2030   Disclosure of Compensation of Attorney for Debtor

 Documents due within 14 days after the filing of the schedules:
  • Local Form 4002-1(B) Affidavit and Disclosure of Domestic Support Obligations THE AFFIDAVIT SHOULD BE MAILED TO THE TRUSTEE AND NOT FILED WITH THE COURT (individual)

 Documents due 7 days before the date first set for the Section 341 meeting of creditors:
  • Copy of the Federal income tax return (or transcript of return) for the most recent tax year ending before filing. THE COPIES SHOULD BE MAILED TO THE TRUSTEE AND NOT FILED WITH THE COURT. (individual) Local Rule 4002-1(D)

 Documents due within 90 days:

 Documents due within 14 days before the date set for confirmation  
  • Local Form 3015-2(A) Pre-Confirmation Certification of Domestic Support Obligations and Tax Returns (individual only)
