• Local Form 4002-1(B) Affidavit and Disclosure of Domestic Support Obligations (individual) THE AFFIDAVIT SHOULD BE MAILED TO THE TRUSTEE AND NOT FILED WITH THE COURT

 Documents due 7 days before the date first set for the Section 341 meeting of creditors:

• Copy of the Federal income tax return (or transcript of return) for the most recent tax year ending before filing. (individual or non-individual) THE COPIES SHOULD BE MAILED TO THE TRUSTEE AND NOT FILED WITH THE COURT. Local Rule 4002-1(D)

 Documents due on or before the 60th day after the date first set for the Section 341 meeting of creditors and before the Discharge can be entered
